I don't have a patio but I have a deck. I liked your suggestion about the lighting. Right now I am attempting my first stain on my deck.
My wife and I love spending time on our deck especially when it is super awesome weather.
What about a big cage with birds, or a small fish pond?
I agree with stevehp, you've got to have some music on your patio or deck too. Some people also love to put plants on their patios, for some plants the patio is great because it gets nice and hot during the daytime.
A built in hot tub is great in colder climates, I have some great childhood memories from hot tub parties in the dead of winter!

Don't forget a patio or garden wall clock! There are some really cool garden clocks, like sundials etc these days, that look quite authentic...
